Participant Waiver and Release of Liability

Effective July 1, 2026

By enrolling in any ZYON Youth Development ("ZYON") program, you ("Participant" or "Parent/Guardian") agree to the following terms on behalf of yourself and your minor child:

1. Assumption of Risk

Participation in soccer training, physical activity, and related ZYON programs involves inherent risks including but not limited to: physical injury, illness, or property damage. You acknowledge and voluntarily assume all such risks associated with participation.

2. Release of Liability

In consideration of participation, you, on behalf of yourself, your minor child, your heirs, executors, administrators, and assigns, hereby release, waive, and discharge ZYON Youth Development, its officers, directors, employees, coaches, volunteers, and agents from any and all claims, demands, or causes of action arising from participation in ZYON programs, to the fullest extent permitted by law.

3. Indemnification

You agree to indemnify, defend, and hold harmless ZYON Youth Development and its representatives from any claims arising from your or your child's participation, including claims caused by the negligence of ZYON staff, except where caused by gross negligence or willful misconduct.

4. Medical Authorization

In the event of a medical emergency and if you cannot be reached, you authorize ZYON staff to seek emergency medical treatment for your child and agree to be responsible for all medical costs incurred.

5. Conduct

You agree that your child and any accompanying family members will abide by ZYON's code of conduct. ZYON reserves the right to remove any participant or family member who violates conduct standards. Billing or refund questions are handled under the applicable ZYD payment policy.

6. Governing Law

This waiver is governed by the laws of the State of Utah. Any disputes shall be resolved in Utah County, Utah.

This waiver is presented digitally during enrollment and must be signed before any participation begins.
