I understand that participation in soccer and athletic activities involves inherent risks including falls, collisions, physical contact, weather, equipment use, serious injury, disability, and rare cases of death. I voluntarily allow my child to participate and assume all risks. I certify that my child is physically able to participate and that I have disclosed relevant medical conditions. I authorize Zyon Youth Development, RAPKAT Inc., coaches, volunteers, staff, contractors, affiliates, facility owners, and representatives to obtain emergency medical care if I cannot be reached. I release and hold harmless ZYD and associated parties from claims, liabilities, damages, losses, or expenses arising from participation except where prohibited by law.

I do not consent to photos or videos of my child being used for ZYD promotional purposes.I have read and agree to the waiver and release above.
