Medical Authorization

Effective July 1, 2026

By enrolling your child in ZYON Youth Development programs, you provide the following medical authorization:

Emergency Treatment Authorization

In the event of a medical emergency during a ZYON activity and if you, as the parent or guardian, cannot be immediately reached, you authorize ZYON staff to:

  • Call emergency services (911)
  • Administer basic first aid
  • Transport the child to the nearest emergency medical facility
  • Authorize emergency medical treatment as deemed necessary by medical professionals

Medical Information

You are responsible for disclosing any relevant medical conditions, allergies, medications, or physical limitations at enrollment and for updating this information whenever it changes. ZYON uses this information to ensure your child's safety and to properly prepare coaches and staff.

Financial Responsibility

You agree to be financially responsible for any emergency medical treatment costs incurred on behalf of your child. ZYON does not carry medical insurance for participants and is not responsible for medical expenses.

Medications

ZYON staff will not administer prescription medications. If your child requires medication during a session (e.g., an EpiPen for severe allergies), please inform ZYON in writing and ensure your child or a designated adult carries the medication.
